Anyone have OSF Health Plans?

I have spent the summer trying for approval.  My policy is the HMO and my pain doctor thought we had enough information but suprise   we dont.  I now need to do a twelve month MD supervised diet with at least four MD visits during that year.  (with other stipulations outlined in the policy of course).

I know many of you have been doing this but I would like to find someone with the same insurance company since it seems that they all have something different included.  I live in the central Illinois area and would be haveing surgery some time at OSF St Francis Medical Center in Peoria, Illinois.

Humana bought out OSF insurance and come January we will be absorbed into there company.
I am so looking forward to another diet where I can loose weight only to yoyo back.  Lisa
