Just passing along an idea....
Every time I go to the doctor or to an ER, one of the first things they want to know is what medication and vitamins I take, then they want a medical history - what hospitalizations and surgeries I've had.
If you aren't feeling well (and why would you go to the ER if you felt fine...?) you're likely not to readily recall what you're taking.
I downloaded a business card template for Word. On one side of the business card I have all my daily meds and vitamins. On the other side of the card is a list of all surgeries and hospitalizations.
Now all I have to do is simply hand them a card.
Now depending on how many surgeries you've had, or how many meds you take, you may have to use quite small print, or make two columns on each side, but just seems like a handy way to make sure you don't forget anything.

This is an awesome idea! I am going to do one for Dale too with all his heart surgeries and meds...too much to remember!
Definitely a great idea. I've used a business card like that for my kids surgeries/medications for quite some time now and it works great. I've also seen some preprinted cards in doctors offices that you just fill in your information. Very handy!
Also a good idea to let your loved ones know where you keep it (or give them one too) in case you are ever in an emergency situation where you can't speak for yourself to tell them where the card is.

That is a good idea. For mine & Bill's stuff, we'd need a magnifying glass to read all of our stuff..lol!
For quite awhile now, we have each kept a record on Word. Mine is 3 pages long...so I don't think I want to try to put it all on a business card..lol. We find it to be a huge help when we go to doctors or hospitals to just be able to print a copy of this off before we go. We each know where the other one keeps it on their computer so we can find the other's in an emergency situation also. It makes it very easy to maintain an up-to-date listing as well. When our scripts change, we simply edit the form and we are all set should we need it.
Information included on my form:
All of my doctors names & phone numbers (and specialies..what I am seeing them for)
Meds: Prescription and over-the counter
Surgeries
Medical Procedures (such as endoscopies, colonoscopies, special tests: MRI, CT, intestinal mobility testing, etc)
Allergies
Adult vacination record: pnumonia, flu, tetnus
On the third page, I have a listing of all of the meds I've been on in the past 2 years to try to ahelp with my intestinal issues...yes, it takes up most of the page.
It seems like I have to add more & more the older I get...

This is a great idea. I use a large index card and list everything on it. I have one card just for all my surgeries, then the other for meds. I just have them copy them at the doc office and give them back to me. Some doc offices do have fold up meds and surg things they give out for free.
But having everything listed on my computer is a great idea too. I HATE it when I go to a doctor and they ask me, "Have your ever tried _____ for your migraine headaches....for example and I think to myself, "YOU have the chart right in front of you in your hands, why do YOU look it up and see if I've ever taken_____? But my charts are always so thick that they don't want to thumb thru all that stuff. LOL

Becky and I use www.roadid.com to keep our information. They offer bracelets, shoe ids and necklaces id and you can have your info for Hospital and emergency needs.
Works good if you can't speak for yourself there is an 800 number that the emergency responce people can call and get your info.
