Yellow Sheet

I did not have my surgery @ St. V's  so I don't know about the yellow sheet, but would love to have a copy of it.  could someone either e-mail me a copy or fax a copy to me?  If so this would be great.

Thank you Terry,   I just got the fax.   Look's like alot of good info.    I am doing pretty good.   Good Luck on Thursday I will be thinking about you,  you will do fine, I didn't  really have any pain.

I've heard the sheets have changed since my surgery.  I'd love to see the new ones.  Think someone can bring them to the clothing exchange?  I wonder if the nuts at St. Vs will give out the new ones?
