Complaint to Ins. commission

Hi everyone.  Another question!!!  I live in Indiana but have BCBSIL insurance.  They have denied me stating no 5 year history (included 7 pages of dr notes from 2001-2004!) and that I have not done 6 months supervised weight loss attempts (no, I have had 2 YEARS!!!).  Anyway, I want to file a complaint with this insurance commissioner, should I file with the Indiana or Illinois Commission?

Is the employer in Illinois...and that why you have BCBS of IL? I would think since the policy is a contract and that contract was written in Illinois, it would be subject to Illinois state laws....so I'd file in Illinois.
