I had my eval done at St. V's Bariatric center. I received a packet from them and there was a letter with info on who to call and make an appointment. If you don't have that info call the center. I paid for this myself. Now if you have insurance and have a specific psychologists you need to see then refer to your insurance policy. One suggestion however, make sure that once the eval is done that they get the letter to your surgeon in a timely manner. It took me several phone calls and frustration to finally be told that I should proceed with my surgeon to pursue my surgery. The communication was frustrating for me.

Pamela, Usually the surgeon of choice has their choice of psychologist that they prefer their patients to go through. Have you have a surgical consult yet? Your first step would be determining who you would like to be your surgeon. If you ask on here, you will be told 'mine's the best' by about everyone on here. lol. We are all particular to the one who did our own. So...how to decide? On the left hand section under Indiana forum (The blue section), there is 'Bariatric surgeons'. Go there and read some of the comments. Also check out the stats on them. If a surgeon has a stat of 98% surgeries were performed 'open' instead of lap, chances are great that they would do yours open. If that doesn't matter to you, then they may be a good one for you, if it does, rule them out. If a particular surgeon does distal switch and that is the type of surgery you are interested in, then include them on your 'maybe' list. Also, check out the bariatric hospital section. Remember that they are a big part of your surgical care team. Once you determine which surgeon/hospital, call their office and set up a consult. Ask them about the psyche eval and if they have a preference of who you should see. Also, ask them what their office procedure is for submitting to your insurance. The more you loop them in, the easier the entire process will be for you. It's pretty common for the psyche eval to be an out of pocket expence. Some insurance companies will reinburse this if they approve the surgery. If you're curious about how your particular insurance fares as far as approvals/denials, check out the 'Insurers' section. Good luck! Sherri
