confused about how to file

Hi, I just registered here even though I have been reading messages for awhile. I now have a question of my own and hope somenoe can help me. I have done things in the wrong order not realizing I would need so many people to agree with me about WLS. I met with Dr. Gupta first and just today met with my PCP, he is wonderful and will write a letter. I am scheduling a phychiatric evaluation with St Vincent's. I just now got off the phone with my insurance company. She reviewed over the phone with me who qualifies and I seem to have those things in order. So she says I can't mail the information myself, I have to have my surgeon download something from the insurance website and fill out page 12 and fax that and all my other stuff to my newly assigned case manager. Will Dr. Gupta's office do this? It sounded to me like they we not going to be involved at all in filing anything with my insurance company. I thought they were going to send me a packet from my initial consultation, is this correct? Has anybody else had to have their information faxed from the surgeon and does the office do this? Thank you anyone that can help

first off welcome to OH . who is your insurance thru. the paper i think they are talking about it connected to the medical policy for your insurance. thats how mine was. vicki

Thank you, My insurance is Blue Cross Blue Shield. I have not ever encountered an insurance problem before so I really don't know why they send my phone calls through something called Empire. Yes, I this paper is connected to the medical policy. Did they file this paper for you? Beth
