I just spent the past couple hours reading about all the letters, evaluations, etc. listed here on ObesityHelp's Your WLS Journey. I think I did something out of order... 1. I went to my PCP and she said she was referring me to Dr. Cacucci's office. 2. Called my insurance (Anthem BCBS) and had a copy of the statues/rules concerning WLS sent to me as well as a Physician Verification Form they require. 3. I called Dr. Cacucci's office, had not received a referral on me, so I scheduled my initial consult. Sounds like according to the site I should have 1. Called my insurance 2. Obtain letters from all my different Doctors 3. Obtain a letter of medical necessity and referral to a surgeon from your primary care physician 4. Visit Surgeon. The Sample Medical Necessity letter on the site is signed by me, but the above says it should be from my Dr. Also is the Physician Verification Form (from my insurance) filled out by my PCP or the surgeon? I just want to cry, this is all so confusing! I just want to be thorough and have a tight case but now I am worried...

Samantha, As I advised Bonnie, call DR. Caccuci's office. Choose the option other than to talk to the nurses. Ask to talk to the office/case manager. Tell her that the process is confusing and that you need some guidance as to what to do. She will be glad to walk you through it. You could also call Anthem and ask them. The physician's verification form is most likely to be filled out by Dr. Cacucci, not your pcp. Hope this helps... Sherri

Its easier than we make it... Just get your original consult with Dr Cacucci and go from there.. she writes the letter of medical necessity.. I just wanted it soooo bad that I pulled out all the big guns and set all the info I could in with Dr. C's letter. Hoping they wouldn't need to waste time asking me for more info.. They didnt. Linda

I agree with Sherri and Linda....there is little you have to do until you are at a point where you have to do it.....let the bariatric center insurance coordinator do her/his job...they usually handle the preliminary things and they will let ya know if you need anything. They are very good about submitting the required documents and they will let you know what all you need. You should not have to deal with the insurance people at all....relaxe...take a deep breath.....and before ya know it....you will be on the other side of your journey....hugs!!!!!!!!!!
