need advice with appeal

ok today i called and got a appeal packet ordered and the ladie told me all i need 18 month documentation, i think i will have that. well she told me if i could get that that the same doctor could go over it and i would not have to go over everythink. well this is the question should i write a letter and i have no idea on what to do

Whinney, I have a copie of my appeal letter. You are more than welcome to follow it. I actually got alot of help from Jan who is here on the Indiana board. She gave me coppies to follow and I put my info together that way. Let me know I can fax to you if you would like. Kathy
