HELP!!!

I need to know if anyone has put a letter together for insurance Re:diet history and execercise program. My dr.'s office suggested I put something together and the dr. can take a look at it. He has had me on a diet and excercise program since last April. I a little upset he hasen't got this toether for me. I went Jan.27th for an appt. to have him get documentation together. When I called the secretary today she said he is out today, and your info is still on his desk. Any suggestions? I would appreciate any kind of letter format to follow..

I would just make an outline of your supervised diet program-have the doctor attach his letter stating the same and that should do it. Attach any letter from your dietitian if you used one. Good luck with your journey
