Sample appeal letters needed

Hi All Can someone post some links for on-line sample appeal letters? We have a few people appealing and they would like to see some examples of what needs to be included besides the Letter of Medical Neccessity and medical records. Thanks everyone!!!!!!!!!

go to yahoo, i have a board about cigna, there we have samples, cignassqueakywheels is the name of it...(please do not use your name here, they have been known to use posts from other boards in the paper work why one was denied) in the appeal they need to ADDRESS the reasons they were denied..QUote parts of the NIH that says diets by morbid obese does not work....i sent in an appeal in a notebook, with tabs, saying what was where....LMAO they still denied me...with all of that... it is not always as easy as appealing, if it were there would be no job for Gary...insurance co will do anything to stall the process...Gary does give out free advise, Gary, has had this surgery and fights for anyone who wants this surgery....there is a gal, named DOODLEBUG is her profile on here, he charged her XX for appeals, ended up being about a 2 year process for her, but he got it done, and never charged her another cent..he is also known to do it for nothing as long as you spread the word of him...he also will work out payement arrangements.... one thing with him...he does the work...insurance so lie to him as much as they do to any of us... in these appeal letters, nothing personal, use only medical reasons, getting off meds ect....it is very very very stressful appealing, people that are approved with no battle has no clue to this process and the stress and all you can think about...live and breathe this appeal...you become so consumed with this...but you have to remember you are in the fight for your life....and what is that worth? this appeal battle is what...and i sit here as proof........though i was finally able to switch ins and was approved right now.... call Gary should you lose this first appeal, try the one on your own, after that get gary, not just any lawyer is gonna work, Gary knows those ins and outs...Gary has been with this board since before we started it, he is awesome...... j

Also, in the book "Weight Loss Surgery For Dummies" there is a sample letter on pages 80-82.
