medicaid requirements for lapband surgery?

So after banging my head against the wall about MY employee health insurance I recieve a letter in the mail about my childrens medicaid, and I have one for me as well. I still have to call my case worker to get cards and find out for certian that I have it but does any one know what the requirements are for indiana medicaid to get a lap band?
how long from initial appointment till surgery date?
what proof did you need that you have been obese for 5 years or more?

I would appretiate any information I can get thank you!

Five years ago when I had surgery I just needed a letter of medical necessity and 8 non consecutive months of weight monitoring. I went in April to see Dr Clark and had surgery schedule by June for the last day in August.

Find out from your caseworker if you have Medicaid or not, & if you do ask her for your ID number over the phone and also ask her for the customer service number (that's normally on the back of every Medicaid card). You should be able to call customer service and with your ID number they can tell you what YOUR requirements are, doctors available,  and any other questions you possibly could have.

I know 5 years ago when I called them, they actually sent me everything they send my PCP to be filled out and documented so that I could help my PCP out as much as possible. They were very very helpful. But that was 5 years ago and I know their procedures have changed but it couldn't hurt to call them and find out.
