Confusion on a Revison

Good Morning all!  I have some questions I hope someone can help with. I had an RNY 5 years ago and never lost enough weight and now I have gained back 50lbs. I had a consultation with Dr Inman  at St V's Oct. 1st for a revison to duodenal switch. She advised me she would schedule an endoscopy to see what was going on and she advised against the switch & reccommended a RNY Distal something or the other (can't remeber to well, I only saw the Dr for about 5 minutes). As of today I have heard back nothing from her office.

I have called & left 2 messages & no one has returned my call...I know they are extremely busy, but 
I'm suprised they have not scheduled the endoscopy. I would think they would want to see if there is anything wrong first before sending my approval request to my insurance company. In case there is something wrong they could tell the insurance co. that surgery is medically necessary, which would be a better chance of approval. Plus I have not scheduled a phyc eval. ...wasn't sure if I need one since I have done this before. I would ask them if I could get them to return my call.

So I guess I'm asking...do I need a phyc eval. with a revision and do you think they are holding back my insurance approval until the endoscopy? I should also mention Dr Inman is having me for 3 months keep a food diary, meet with the dietician and attend support group meeting. Any feedback would be greatly apprecitated!! Any answer given here is not based on medical fact.  I guess what I am trying to say is let the DR. or DR's do what they do.

One of my fears is am I going to go back to old habits and gain weight back.  I have made up my mind that I have to exercise more and only eat foods that are good for me and in portions that will not add calories.  It is still all about the calories eaten vs. calories burned.

Bypass surgery removes the hunger feeling.  I can still have head hunger which is when I turn to doing something else rather then place food in my mouth.  I have a daughter that is 4 years out from bypasss surgery and would love to lose 20 more lbs.  She has not put on weight during that time so she is in a neutral state.

She knows this is good and will work out getting on a calorie plan that allows her to lose whatever and whenever more she chooses.

I have Dr. Inman. On her answering machine system, there is some kind of option to talk directly with someone. I think it's option 2. It also tells you later, if your call hasn't been returned in a day, you can choose that option. I would call back, and do that, and just explain that no one's returned your call yet. I think her office is a bit busy, but just be a bit of a squeaky wheel if you need to.

There isn't a hard and true answer to your question about approval/psyche eval/etc. Part would depend on 'why' a revision is needed. I had to have one when I was 6 months out because my stoma kept perforating. That was a medical neccesity. Approval, no problem. Psyche eval, not needed. For someone 5 yrs out though, you may have to jump through the hoops like someone who never had a WLS. I would also thing that an endoscopy would probably be done prior to any next step being done since she would want to ensure that the failure doesn't have a physical cause (ir stoma too big, staple line failure, etc)...which is why the food journal as well..to ensure that you are using the tool correctly as the best you can.

I would follow Linda's advise and call them as many times as it takes.
