Any medicaid patients here??? PLEASE help me

Im so confused!!! When I got my packet, I filled it out.  this mind you was in 05.  I had to keep it becasue I foudn out I needed 5 years of records.  I had to make calls on this.  I had to get a new doctor and only had 3 months with him.  My old one i had around a year, and one before her was around 1 year..so I told her I have around 2 1/2 years.  She asked if I had any notes from my doctor.  I said I had one but it was from a previous doc.  She said its ok just send it in.  I then asked her about the 6 mnth sup diet.  I told her that I was on and off of my depression meds, I took mirgrain meds, i was Diebetic, and so on.  She then said that as long as I had my note saying that I needed it, I was ok.  So I sent in my packet.  I called her about a week later to check.  she said I was eliagable, and I asked what she ment.  She said that I qualified for the band.  I said ok, but will mediciad cover me.  She said yes, they would.  I said how do you now?  she said that becasue I have co mobilities, and my BMI is over 40.  I said, but i need that 6 mnth diet.  She said that as long as I have that note im good.  I hung up thinking I was good!! Well..I had my first app aug 24.  I talked to Dc allen.  NICE GUY!!! anyway, he said that i do qualify, and gave me a list for medicaid to cover my ass.  He said to do what was highlighted.  So I go to my doctor, and give it to them.  they ask if they want everything on the list.  I said, well..he said to get what was highlighted.  they then want me ot make sure, so i did.  The insurance girl, whos new, said for me to get everything.  I said, then why would allen tell me just to get those 2.  She said that for med to cover, I needed to be clear.  I said ok.  I told my doc, and he said ok.  Long story short...i end up getting a EKG, which isnt what was ordered, but it was fine.  I was cleared the next day but it ook 2 weeks for my PCP to fax it!!!! As of Friday, its been a week since htey where faxed over.  I dont even now if my papers have been faxed to my insurance, but I did get a letter a few weeks back saying that she needed my clearance ASAP.   OHH then the insurance wont return my freakin messages!!!!  Her sup did but appearently didnt listen to it! she called my house when I said to call my cell, and she asked what I wanted!!!!
