Looks like I will have to start my 6 month supervised diet over again--advise please
Don't give up! This is just a bump in the road. I have Anthem and I didnt have to have 6 mos of supervised diet. I only had to have 5 years of med records. I see a nurse practioner and she is cool and does whatever. This is bad, but she just wrote in my chart that I done the 6 mos. diet and I made up weigh in days on my own paper and weights and gave her a copy and she said she supervised it....lol but I didnt need it anyway so it didnt matter. I have Anthem Blue Access, what do you have?
Anthem has a form called "surgery for Morbid obesity, Physician verification form". Call the insurance and ask specifically to have one faxed to you. Take it to your PCP. It asks them yes or no questions.
1. Has the patient been MO for at least 5 years.
2. Has the patient failed at least 6 months(mine says 18 months, because that was the requirement in 2005) of physician supervised weight loss treatment.
3.Outline previous physician supervised conservative therapy, and a couple other questions.
My PCP was not good at mentioning my weight in his notes. He filled this out and marked yes to the questions, then wrote that he had tried, wt. control meds, and supervised diets. I never sent in any physician notes, and was approved quickly. My PCP also wrote a letter. I found a letter on this site and took it with me to the doctor. You need to tell your PCP what you want. I was approved in 1 week, but they needed a surgery date and length of bypass. Dr. C. sent it in, and I was approved then.
If you want, I can fax you my PCP note, and the form. If that will help. I have blue access also.

Yea that is what I had, is it called a provider tool? My PCP filled it out and submitted it to my insurance co and I was approved. Then the surgeon's office had to do it again to add themselves as the surgeon and where the procedure would take place. I didnt have to send anything else in either, just the doctor filling that out was sufficient.
I thought of something else I did. when I went to the initial meeting with Dr. C. you had to fill out a weight loss history form. I wasn't prepared at that time so I just kind of guessed. When they sent me the packet to send to insurance. I called them and asked them for another diet history form and I started with the most recent and went backwards, not leaving any gaps in time. I'll fax that to you also.
