Insurance weight management question
Ok, I have been reading and reading these criteria since I got them in the mail thursday, and I still can't understand everything. I hope someone here can help.
Here is what they want for the weight management history. ALL of these:
a. Obesity duration greater than 5 years with review of weight management for at least the preceding 2 years. What exactly do they mean by review of weight management?
b. Participation in a weight loss program for 6 months (within the past 2 years) with chart notes that include documentation of ALL the following: here's where I get confused
1. A dietary program for weight loss which consists of a very low calorie diet program (eg Optifast, Medifast, Nurtisystems, Jenny Craig, Weigh****chers ) whi*****ludes monthly weigh-ins and nutritional analusis. I have never done any of these other than weigh****chers and it was only online where I track everything myself. I have been on a diabetic diet for the past 4 years but only saw the nutritionist once or twice to explain it to me. So am I covered here?
2. At least monthly clinical encounters with a healthcare professional who does not perform weight loss surgery. I have this, although there are gaps in there from lack of insurance, so does that matter?
3. Increased activity/exercise regimen unless contrainindicated. For this I have my own records and that is all.
4. Behavioral modification program supervised by a qualified professional to reinforce dietary therapy and increased physical activity. So will my 2 visits with the nut. count for this??
5. Weight loss management history includes pharmacotherapy with physician prescribed weight loss drugs, or documentation of why pharmacotherapy was not tried. I have this one.
The rest I understand fine. I don't know why I can't figure these out. If anyone can help me at all, that would be great. Thanks
