for people that had to do 6 month diet for ins company

my ins company wants documentation of supervised nutrition, exercise and weight loss program lasting at least 6 months. I got most of my paper work from the dr that i have been doing this with and I dont know if I have what I need. what needs to be on the documentation? if any of you have one that i can look at it would be great. brandy

Brandy, I had to do a six month diet and exercise program. I visited by doctor's office every other week for weigh-in's, saw a dietician once a month, went to monthly support group meetings, and joined Curves. So, I had copies of the records the doctor's office kept for my weigh-in's, copies of the records of my visits with the dietician, copy of records showing my attendance at the support group meetings, and Curves provided a copy of my attendance records there. All that did the trick. Once the paperwork was received by the insurance company, I was approved in about seven days (this after being denied and an appeal being denied a year earlier). Good luck!
