have any of you need all this for the ins company
here is the list my ins company gave me and said that i need.
1) documentation of morbid obesity that had persisted for at least 5 years.
2) current BMI , height and weight
3) letter of medical necessity from pcp with documentation of treatment for morbid obesity and co-morbidities.
4) letter of medical necessity from surgeon
5) detailed weight management history
6) documentation of completion of a physician supervised nutrition and exercise program. working with pcp and nutritionists. for 6 months
7)psychological evaluation within the last 6 months.
if so how did you get all the info that you need. my dr isnt really doing his part.
I have # 7 done they send my paper work to the surgeon I am going to see him tomorrow so i can get him to get me a letter and he will fax both of them to the ins company but my pcp dr hasnt send the ins company a letter from him so i am going to write one and get him to sign it and send it in on my own with #1 and 2.
so that leaves # 5 and 6 how do i get that info I am almost done with my 6 months of diet but I dont really know what the ins company wants. can any of you help me with letters that you did or anything to help.

I had my drs give me 1 yrs worth of wts from all my appt's Between my family dr and my gyn I had several wts. Then I put them together with the ones I had from my health club and sent that in. I also sent record of all my visits to my gym and what my workouts were each time I was there. Plus sent a note from the guy at the gym how long my membership had been in effect.

I requested a copy of my physician's office notes. He also wrote (after MUCH pestering from me!!) a letter for me to include. I included a letter from the employee health at work with a record of my weights since my employment, an Excel journal of my weight/food/exercise (for 6 months), dietician notes, medical testing copies (ie sleep apnea). Just get all you can, copy it all, and send it all in. I included cover sheets to separate topics (ie 'Physician letters', 'personal journals', 'medical testing', etc) and a cover letter. I figured the more organized I could make it, the easier for them to read & make their decision.

My drs were pretty cooperative. I had letters of medical necessity from my pcp (both current and former), gyn, pulmonologist, and the sugeon.
My insurance company wanted the 6most supervised diet and it they had to have the info from my doctors medical notes which he took at each visit. I gave them so much info and then was denied due to lack of 6 mos diet. I appealled and gave them much of the same info again along with a drs notes back from at least 20 years...and they approved!!
My suggestion....give them anything you can get your hands on.

Surely when you went to the doctors over the last 5 plus years you weighed in from time to time. That is what your insurance company is looking for- documented weigh ins over atleast the past 5 years (one weigh in a year is sufficient) that you are morbidly obese. I got 10 years worth of records of weigh ins at 3 different docs and sent that to my insurance company. I also had my co-morbids documented too ie: sleep apnea and high cholesterol.
I took my docs records and created a one page summary sheet showing my BMI and weight every year for 10 years so there was no question as to if I qualified. A couple of years I had no weights for... I was still approved in 8 days!
This is what I sent in, copy and pasted from my profile:
1. The first page is a letter from me explaining my situation and what information I have included in the packet.
2. A letter from my PCP reccomending surgery- its a whole two paragraphs long.
3. 6 months worth of my "medically supervised weight loss" medical records. I can see where that might take maybe 20 minutes to get through if you were combing through it. Still, my PCP uses a laptop, so all the notes are neatly typed along with all my information for that visit.
4. My letter from Dr. Caccuci. Two pages long, a very easy read.
5. I had to show that there is a support group in place for where I will be having surgery, so I included the schedule sheet for those, nopt like you have to do a lot of reading on that.
6. A diagram of the proceedure.
7. A letter from a nutritionalist stating I've been advised and understand the life style changes necessary.- 1 page.
8. I included the meal plan sheets with the nutirionalist letter, so 6 pages there. Also an easy read.
9. Pulmonary dr approval- 1 1/2 pages, typed easy to read.
10. psych eval- had written and a little taxing on the eyes, but legable nonetheless. I think it was 4 pages long, so maybe a 15 minute read at the most.
