SIHO INSURANCE

Im from Columbus indiana and i'm trying to get the surgery with dr. David Lee. I was wondering if anyone else has been to him and if anyone has SIHO insurance and if it is easy to get approved with them and what they require for u to do before surgery or if anyone has gotten lucky and didnt have to go through months of dieting before they approve you? Please help i need to know if Siho is good about approving the surgery or not Thanks so much. :-( :'(
