Submitting for Approval through CIGNA
I really can't offer any words of wisdom regarding Cigna, but the one thing I would say is that if at first you don't succeed, try again - appeal - do whatever you need to do. I was first denied, appealed and denied again. Found out what they wanted from me, did what they wanted and submitted again a year later - alas Victory. I was approved and my surgery is Tuesday.

I have CIgna and got approved in about 4 months... There is a form that you need to have your doctor fill out that documents your 6 month weight loss program. There are also like 6 specific questions that the dr needs to answer in each section for them to not question anything. And you need to ride their ass!!!! I called them 2 times a week... Send me a private message with your email address and I will send you the documents that I used to get approved.

I just had my conference call with CIGNA yesterday and they said I should receive an answer by mail within 5 days. This is my second appeal. They approved me for surgery, they are just denying the DS procedure because it's "experimental/investigational." The last appeal packet I sent them was 109 pages of case studies and Medicare documentation showing they now approve it. So... we'll see. Form what I've seen and heard, just make sure all your ducks are in a row and if you get denied just appeal, appeal, appeal. OH, and threatening to involve the Indiana Board of Insurance doesn't hurt, either.
