PCP letter

Okay,, becuase this is my first go at this, and I'm not sure if this is good enough can you guys help me out. I got my letter from my PCP and I don't know if it covers enough,, or if it'll work. Any advice on what i should have covered in it.. She has all my medical conditions and labs in the letter but that's about it..
