1st denial, but I think I can fight it!!!

I recieved a letter from Anthem today that was dated 6/16/06. It states that the services are not medically necessary because the following: As required by Anthem Medical Policy, there is no documentation that the patient's treatment plan includes pre- and post-operative dietary evaluations and nutritional counseling. Well, I just had that yesterday 6/20. Why would Dr. Clark's ofc send info to anthem for approval when they didn't have everything? I work at a doctors ofc and I am the surgery scheduler. I would not dare waste my time or the patients time if I didn't have all the info. Sound Silly!!!!

not sure that is done till later, or once you have approval, so this could be something new they are asking for that the office may not have known???
