Has anyone in Indiana had medicaid pay for their surgery? What are the requirements to get enrolled for medicaid? (I have a low income, but no children) How do I ask for the surgery when applying? Danielle

I have Medicare and Medicaid, I do not have children. Your primary Dr maybe able to help you. Medicare requires a hospital of excellants and the only on in Indiana is St. Ventancents that I am aware of. Medicaid will pay for the surgary starting July 1 st. They require that you do six months in a row with your primary Dr. and a dietion. I am not positive but I believe that is one of the new changes that have been made. Good Luck! hey Danielle, I had Medicare and Medicaid pay for my wl banding. All you have to do is call the 1-800 customer service number on the back of your Medicaid card and tell the rep that you are interested in WLS approval. They'll notate your file and send a packet of the requirements and forms to your PCP and you can ask for a copy of the guidlines/approval list to be sent to you also (at least that's what I did and the process went very fast after that). Thanks everyone. I don't have medicaid yet, but I have been advised that I am probably eligible. Where do I go to apply? Is there a 1-800 # for eligiblity? Do i have to have a disability? Danielle

Contact your local Division of Family and Children Services (aka welfare office) - you fill out the application there.

I'm a caseworker in wabash county. All you have to do is go to your local DFR(Division of Family Resources) and fill out an application for Medicaid. Your caseworker will take it from there. There are guidelines you must meet in order to qualify. If you would like to PM me, I can give you those guidelines.

I picked up the applications for assistance today. My problem is that I have a lot of people that I live with, do I have to put them down if I am just applying for me? My family applied for Medicaid and they were denied because they made too much money, (last year sometime) will the same thing happen to me because of this? Danielle
