My PCP's Letter of Recommendation...
I wanted to post the letter of recommendation my PCP wrote for me to get some feed back and to perhaps other people currently compiling their packets. I saw this letter posted on the WLS in your 20's message board and decided to print out a copy for my PCP as an example of the type of letter I would need. She proceeded to add to it etc. The person who posted it originally said they were approved within 10 minutes because of their letter (I wish!) Here it is (Guess you will all get to know a lot about be but I have nothing to hide )...
To Whom It May Concern,
I have been Samantha Fox's primary care physician since November 2004. She has suffered with obesity since the age of 8. She has tried several medically supervised weight loss regimens including Adipex, Wellburtin, Weigh****chers, and low 1200 calorie diet for greater than eighteen consecutive months. She has also tried exercise programs including cardio, weight lifting, and water aerobics, all with insuccessful results.
Samantha Fox's obesity has caused or worsened urinary incontinence (625.6), Arthralgias (719.49), Migrain headaches (346.10), lower back pain (724.5), peripheral edema (782.3), and menstrual irregularities (626.4). Continued obesity will likely also lead to hypertension, diabetes, sleep apnea, hyperlipidema, arthritis and other life-threatening conditions. Not only will these conditions shorten her life span but they will also result in expensive insurnace treatments.
Her current weight is 272 pounds, height 67 inches, and BMI 42.6.
I have discussed bariatric surgical procedures and risks involved with her at length. she understands and will comply with post-surgical requirements. I recommend she undergo Roux-en-Y Gastric Bypass with Dr. Brenda Cacucci. I feel this is a medically necessary procedure for her morbid obesity.
