You send it to Sue, Dr. Caccuci & Dr. Gupta's nurse. Her email addy is: [email protected]. FMLA paperwork has to be done by the surgeon so Sue handles it, not the other office staff. That's how it was for each of my 3 surgeries through them anyway.

Oh..forgot to add..I've had FMLA paperwork done 4 times by their office (3 for the surgeries and 1 for an intermittent leave for my many, many endo's)..I think I was only charged one time. So....shhhhh...if you don't offer up, you probably have a good chance of not having to pay anything at all.

I just got off the phone with Dr. Gupta/Dr. Caccuci's new nurse. I didn't catch her name. She let me know that Sue had left..but she didn't know what her email addy was yet. She's suppose to call me back tomorrow (Dr. Gupta is out of the office today). Hopefully, she'll know what her addy is then. In the meantime, you could call their office and choose to talk to a nurse..then to talk to Dr. Gupta/Dr. Caccuci's nurse. She'd be the one to handle the FMLA stuff. I think the time I was charged was because I had brought it in with me during an office visit and it got put in with the paperwork at check-out. The other times, I faxed them to Sue.

Why did you have to have so many sets of paper work filled out? On mine I am requesting 2 full weeks off and then back to work for 2 weeks only half days. I also requested to use the leave for various follow up appointments. I will call here in a few and get the name etc. Now you got me worried I need more forms filled out!! haha

You never read my profile? Makes for some interesting reading...especially for pre-ops. It kinda lets you know some of the complications that can happen with this surgery. Fear not though. Most people might have to have a few endo's during their journey but it's not 'norm' to have a laundry list of stuff to happen. I had to have a new set of FMLA paperwork for each surgery (RNY, RNY revision, and Hernia repair...AKA..Kinky bowel syndrome). I also had another set for my intermittent leave which covered the days I missed for my endoscopy's (15 of them...strictures & ulcers). Since not everyone has the list of stuff I have, you'll most likely only need one set of FMLA paperwork.

Most FMLA's are for a set time for a set reason...such as the 2 weeks off and 2 weeks part time that you are requesting for your RNY. Which...I take it your job isn't very strenuous..? That FMLA request is only good for that surgery. If you want FMLA approved time off for your doctor appointments...or if you are unfortunate enough to need multiple endo's like me, then that is a separate FMLA request and is called an "intermittent FMLA leave". Where I work, they are usually set up for a year with XX number of days off per week/month. If you were to need your gall bladder out next October, then that would be another, separate FMLA request. Make sence? "Time" has nothing to do with how often the paperwork has to be done (unless it's for the intermittent one).
If/when I have my little rear-end problem operated on, that will be an FMLA submission. My foot surgery will be another. Each time I'm off for PS will be another.
