Hi Jessica.Did you make it to Dr.Shayani`s Seminar in Chicago the 10th?I did and The next day when I went for my consult all they asked for on the paperwok I had to fill out was what methods I tried in the past.1.medical methods-I listed that my Dr. put me on Straterra for add but loss of appitite was a side effect rather than a weight loss tool.2.Like what types of programs you`ve tried.I listed things like slim fast.dexitrim.Richard Simmons(the one where you have to close a window every time you put ANYTHING in your mouth

)Starving myself,Eating less,excersizing.He looked my paperwork over and didn`t say anything about having to have anything documented.
