The doctor we are using does not take the IL Medicaid card but the Hospital does, but they say the doctor needs to submit the approval request. My doctor is unsure how to do this. Does anyone have a form or a phone number for IL Medicaid for submitting surgery requests? Please help!!!1
Go http://www.hfs.illinois.gov/ the number should be there somewhere. If you surgeon does not take medicaid he wont be able to submit a claim unless he's an approved provider.

I don't know but THe first Dr I wanted I called them and trhey told me the hospital is covered but I would have to pay 6000 up front for the dr. good luck to you and I hope someone has better help for you then me!
