Sample PC Recommendation Letter
Does anyone have a sample of a primary care doctor recommendation letter? I think I will have more success with my PC if I bring him a letter. He is generally opposed to giving any referrals so I want to make it as easy as possible for him to refer me. If not I will change PC docs. Any suggestions for AETNA POS in IL?
I have Aetna Pos and I don't need any referrals. I can go to any doctor in my plan. My surgeon and his hospital happened to be in my plan. I didn't go to the surgeon my PCP recommened. I didn't like his staff. I did my research....found out the surgeon I wanted was in my plan, and made an appointment. With POS no referrals are needed. You will need his recomendation once your information is submited to Aetna.
Below is what I used; I brought my laptop and a portable printer into the Dr's office so I could have him sign it and be on my way. Hope this helps!

The patient also suffers from urinary stress incontinence and lower back pain as a result of very severe obesity.
On June 10, 2004, ____________ was hospitalized with fluid in the lungs. At that time she was diagnosed with anemia, diabetes mellitus and hypertrophic cardiomyopathy. The patient was put on short term disability and returned to work after 4 weeks. On December 21, 2005, the patient was again admitted to the hospital after suffering a severe asthma attack. The patient was put on short term disability and returned to work after a 4 week period.
_______________ has consulted with a nutritionist and psychologist for guidance in her weight loss. She has also continued to follow good diet habits and participates in exercise programs when it is physically possible for her to due so. Due to Ms. ___________ medical conditions, the impact of her weight on her health, and the continued failure of conventional weight loss treatment plans and support, it is my recommendation the patient seek gastric bypass surgery for the well being of the patient.

This is the letter my dr gave me.......

I am writing this letter in regard to [patient name], who had been my patient for [time frame.] I have treated the patient for [disease name] with [treatment stragety], and [disease name] with [treatment strategey] (etc.) [Patient name] is clinically morbidy obese, and presented with weights ranging from [weights.] The patient attempted a low calorie diet under my direction from [dates], as well as a course of [medication] from [dates]. The patient lost x pounds] during the course of the therapy, but did not maintain the loss through medical managgement. The patient tried commercial diet plans: [names of programs], but was again unable to maintain weight loss. It is my reommendation that [patient name] pursue laparoscopic gastric bypass surgery for the treatment of morbid obesity.
