Doctor Supervised Weightloss

For public aid how much diet history do they want? When I went to the seminar that my surg. gives they said not to send anything over 3 years old. Most of my doctor supervised stuff is older than 3 years. Since December I have been seeing my doctor 1x per month discussing diet, exercise, and my diabetes. My weight has flucuated up and down the last few months, with no real change in my blood sugar. He documented that I am trying to watch carb intake and walking. Anything else that should be in my documentation? Shelli

Unless something has changed public aid doesnt ask for a diet history or atleast not a documented one.. Just list what you were on or did and about when you did it.. Debby
