Sample letter

WLS Family, I am hoping to have Dr. Tiesenga perform the Lap-Band surgery. However, I am have having problems writing my ESSAY. I have written several (3) letters in the past few months; none seem to be acceptable. If someone would please assist me with the letter, (samples) I sure would appreciate it. Thank you in advance! Juli, I am also a patient of Dr.Fred, I have written my essay all i did was basicly answere the questions that came in with the packet that you got at the first intro meeting with Cecila. The question are.... 1)What are the side affects of having surgery? 2)What do you have to do to have a successful outcome ? 3) What do you anticipate from this surgery? Just speak from your heart. How you feel about all that this surgery will bring to you the highs and lows. Explain what you will be bringing to the table with this surgery. The Dr. is going to give you the tool .....So now what will you be doing to help yourself along. this is what thay want to here. I had a copy of my essay with me when I went for the Psych Eval and she read it and said it was great. It all came from My heart I told her. Juli, I hope I have helped you.
