Mrs. Lora Orr appeared on time, appropriately dressed and groomed. She was co-operative and appropriate throughout this evaluation. Mental Status did not reveal any problems in memory, judgement or fund of knowledge. there were no indications of hallucinations, psychosis,or organic brain dysfunction. The Beck Depression Inventory did not show responses indication significant depression.
Mrs. Orr stated that obesity has been a lifelong problem. Even as a grade school child, she struggled with being overweight. She states that she may eat the wrong foods, but has never binged and purged as a way to keep her weight in check. She has tried numberous diets, just to gain everything back and then some when going off the dietary regimen. She denies eating for pleasure. Right now she weighs xxx pounds and is feeling the effects of this weight in her body. She states that she has diabetes, hypertension and joint pain. Mrs. Orr is hoping that this surgery will prolong her life and give her more energy and pleasure.
Mrs. Orr is married with 3 children. She and her present husband were married in 2001 and are getting along "great". He is supportive of this surgical intervention and is prepared to see her through post-operative phases. Her mother and sister will also be available durging that time to help with household and the children.
Mrs. Orr states that she has researched the procedure for several years and is ready to have it done. She is aware of possible post-operative complication, whi*****ludes death, infection, embolism, dumping syndrome. She believes that she can follow recommendations regarding her care without difficulty. She knows somebody who weighed much more than Mrs.Orr and had a very successful operation, which she finds very encouraging. Mrs Orr also agreed to go to the local support group which meets monthly to inform herself as to how others have fared and are dealing with the operation and post-operative lifestyle.
Mrs. Orr takes Effexor 150mg due to anxiety. She also takes medication for hypertension, diabetes and a diuretic. She has never been hospitalized for any illnesses. She denies drinking alcohol and smoking.
In summary, I believe that Mrs. Orr has the mental competence to understand the risks and instructions and is capable of making this decision regarding her surgery.

You gotta love psychiatrists "She denies drinking alcohol and smoking" as if you were accused of it, and had to defend yourself...
Its just funny...the wording...
But it looks very positive! Certainly seems like a resounding YES, She will succeed, from an expert, no less!
Good luck, Lora...one more hurdle cleared!!!
Have a good day!
