Elizabeth, I know some have done a table, showing the full history of their weight loss attempts. I am including an example of what it might look like below: Diet Dr Supervised? Dates st wt end wt #s lost Atkins yes 02/02 - 02/03 240 230 10 Grapefruit no 01/01 - 01/02 240 240 0 Low Fat yes 01/99 - 08/99 265 240 15 etc., etc., If you go back as far as you can remember, it seems more like you have been trying, even if unsuccessfully. There are some profiles out there that have examples, browse around a little and see what you can come up with. Good luck! Jennifer

Elizabeth, You may find, after meeting with your surgeon, that his office is set up to help you deal with your insurance company about this. They are, after all, the experts. When I went to my surgeon's office, they explained that they have a department dedicated to insurance approval. They gave me a thick packet of information with forms to fill in detailing all my dieting history, all my co-morbidities, family health history. It took a while to fill out, but using that information, they were able to write a letter that got me approved. Right now, just worry about getting the facts down on paper. Document the what/when/how's of your dieting history. The format you can deal with later and you may discover the surgeon's office already has it set up for you to fill in the blanks. You don't have to arrive at your surgeon's office fully approved. Just get all your ducks lined up so when they ask, you'll already have it. Just take a deep breath and keep chronicling your history for now. The surgeon's office will let you know what to do next. I promise. Stef
