I need help writing a letter to insurance company!!

I was told by bc/bs of illinois, I was to submit a letter stating my weight loss attempts, why they failed, why did I gain weight back, and so on. I have been on and off diet pills, for the last two years, but I don;t like the way they make me feel. Actually I have taken them on and for the last 15 years. I also bought phentrmine on line, when I did not feel like going to the doctor. Also, I have tried the low fat, the atkins, exercising I bought a treadmill and gazelle. I stopped eating meat. I tried EVERYTHING. Even those new relacore pills at walmart. Metabolife, ephedra,and ephedra free. I tried living on salads. I did lose some weight, but when I tried to eat NORMAL, the weight came back. Diet pills don't work, anyways, they just make you crabby and they would make my heart have pains. I get shortness of breath, alot, lower back pain is terrible, and so on, also, I am to submit to the insurance company a letter from my family doctor a letter stating he supports the surgery and why? I hope he does, he was not in the office today, but I showed his receptionist the records from his office on when he prescribed the diet pills, and their discussions with me on weight loss, and diet. I will know tomorrow if he writes it. (hopefully) Also, my appt. with my surgeon is Thursday, wow, I got in for an appt, it only took 3 days. The insurance co. said it will only take a couple days to approval, and they said they allready started a chart for me, a reference number and a caseworker, they said they want to help me along, I was hoping to have this done soon. So, we will see. Well, anyways, can anyone help me word these things to the insurance co? I need to get accepted!! I guess with this surgeon I need the psych appt. do I make an appointment with a psychiatrist now, or wait? does the surgeon schedule the cardio, pulmo, and whatever? thanks to everyone in here!!!
