Help Seeking out Doctors!!

Not for WLS (yet), but for a WLS friendly General Practitioner, and Ophthamologist, and a Podiatrist. I've not been to a doctor for about 3.5 years (since my last check up after my C-section) I've had chronic foot pain for the last few months, where I can barely walk in the morning or after I've been sitting down for a while, so I went online to Mayo's website and learned about Peripheral Neuropathy, which can be caused by diabetes. (I had gestational diabetes with my 2nd/last pregnancy 4 years ago. My last blood sugar test, soon after the pregnancy, was normal, but gestational diabetes puts me at a 40% higher risk for developing Type II). Now I'm worried. What kind of doctor do I go to to test for diabetes?? Then I read through Mayo's diabetes section, and read about how diabetes affects vision, and mine has noticibly worsened in the past year. So, now I'm really worried about diabetes, and want to go to a doctor to either confirm it so I can be properly treated, or rule it out. And I want to find doctors to help with my foot/knee pain, and check out my vision. How do I find a doctor? I'm new in the area.
