I need help with my diet hist. for Blue Advantage HMO of IL (BCBS)

Hey all, I've had all of my prep tests done and everything checks out, but I'm having trouble getting my dietary history together. I was told only recently that I needed to have 1 yr of dietary supervision by a doctor and am having some trouble getting my records from a previous physician. What I need the most help on, though, is exactly what should I put down, how, and from how long ago? I don't know if I'll even have time for the approval I need, b/c I heard here that BCBS will no longer approve the surgery as of JAN 1st! Help!

I did my diet history starting at birth but seriously i dont have bcbs and they are pretty rough on this subject so I'd put down everything and not leave any holes for them to open up.. Debby

What do you mean by "holes"? Do they look for a continuous dieting pattern, do they pay any attention to self-imposed diets? What exactly do they want to see, and I can probably provide it to them. PLMK! --Amber

I sent information from as far back as I could (15 years?). I was able to get some medical records, and some I couldn't. I sent BC/BS (Federal plan) pages of medical records, cancelled checks, receipts, photo copies of old pill bottles (I still had the old empty pill bottles for some reason), weigh****chers records, copies of Medifast boxes, etc. If it could have pertained to my struggle with my weight, I copied it and sent it. My packet was almost 100 pages. I was approved within 5 days. Good Luck. Lori
