I see a lot of looks but no replies.
Unfortunately, I do not have health alliance either. BUT, I strongly suggest you either go to your insurance company's website or call the 800 number and get a copy of the requirements for weight loss surgery and what is covered and what is not. Every person's policy is set up dependent upon their employer's requirements. Someone else may have Health Alliance and have totally different coverage than you.
It is YOUR responsibility to make sure that you meet all the requirements that your policy asks for. This is critical because if you do not, you will be denied. Insurance companies will look at everything with a fine tooth comb to see if they can get out of paying.
I had my request rejected the first time around due to some technical issues. Many others have also had problems. Dont let that discourage you, keep plugging along!

Hi there! I have Health Alliance (H.A.) for my insurance. As soon as I started considering weight loss surgery (WLS), I dialed up the phone number for Health Alliance and asked for the person in charge of the “I Can Change" Program, realizing I met their guidelines as a candidate for WLS. You can also go to the Health Alliance website and look up their policy for bariatric surgery.
I had to do six months of telephone coaching and lose a percentage of my body weight before being allowed to have my (RNY) surgery. I had to have my primary care physician (PCP) document my starting weight and send in paperwork to Health Alliance and then at the end of the six months, the PCP had to document my weight again to see if I’d achieved that percentage of weight loss (and maintain the weight loss up to the date of surgery).
Your main thing is to follow Health Alliance’s rules – check to make sure the doctor(s) and hospital are on their approved list. Each surgeon’s office is different as to what they require before surgery (mine required a Bod Pod, nutrition classes and support group meetings, pscyh eval, EKG, bloodwork, etc.). So you’ll want to see what H.A. will cover. I kept a binder documenting every date and person I talked to as there is a lot of information being given to you.
