NW Suburbs --Psych Eval Referral?

Can anyone point me towards who they used for their psych evaluation?  I have BCBS and that is the one piece of the puzzle I have yet to schedule.

I would think your doctor has someone who does the evals for the program. Are you going to a Center of Excellence for Bariatric Surgery? Most of the programs are self-contained in that the psychologist, the dietician, etc are all part of the program.

Otherwise, I know a psychologist in Gurnee if that is in your area.
