BCBS IL requirements

I am trying to find out what exactly people have turned in for the  6 month diet requirements.  anybody have any info???

You should call your insurance company and have them fax/mail the exact requirements.  Every BCBS IL plan differs from employer to employer.  I have BCBS MN PPO and my insurance sent me an copy of their requirements.  It's also good to have this because I heard of someone saying they were told WLS was covered, just to find out later it wasn't.  So get a copy of the requirements in writing.  Good luck.

The form I had to fill out for my BCBS IL was for the last 4 years and I just listed all the times I went to WW and also tried by myself with diet and exercise.  That worked for me.

I want to echo what was said on this thread. All BC/BS are not the same. My BC/BS of IL plan could be different than yours.

It depends what YOUR company negotiated for its employees. You will need to call your insurance company (number on card) give them your ID and group number info and see what was negotiated. Take notes, take customer service people's names. Ask them to email/mail/fax you the plan description so there is no mistake.
