Usually the WLS submits the info cause they know what to send and what info the insurance company wants. Doesnt your WLS have a person or persons that only deal with insurance issues? I would call them and find out why they did not submit for approval. Who is asking for the HPylori test and why? That is something easily taken care of but there should be a reason why you need this for WLS?? Seems fishy to me. That may be part of another problem you are having but how does it relate to the WLS?
I would check with your WLS office and see what is up. Make sure the answer all of your questions before they hang up.

Good luck with the process, Randy. Hopefully, your doc will get it right the second time around.
I too have to have an H. Pylori test done. This bacteria is responsible for a large percentage of one type of ulcer. If a person tests positive, a course of antibiotics will eradicate it and reduce your chance of forming an ulcer that is difficult to treat after surgery.
