Can someone please assist me?

I am trying to figure out a estimate of out of pocket cost in-network BCBSIL PPO in-network provider.

I'm not sure anyone can help you on this one, as everyone's insurance coverage is so different. I would call Alverdy's office and your insurance company directly and ask them to walk you through it, step by step. It is their job to do this for you. Don't be afraid to call them.
