how do I put type of surgery under the atvar

I had my Lap band surgery on 12/02/08. After lurking here for 6mo I'm ready to start posting. I am missing my type of surgery under my atvar, and I can't figure out how to add a ticker. Can anyone help me?

For your ticker, copy the picture of the finished ticker not the codes.

The surgery date is found under the MY OH section. There are a series of questions to fill out and one of them is your surgery date.
