It must be that Tricare hasn't paid in the past.  I have United Health Care, got pre-approved and didn't have to pre-pay Dr. F. anything (and he is out-of-network).  UHC hasn't paid him yet (probably because they submitted using my husband's last name and not mine), but at all my pre-op appointments they said UHC was good with bypass surgery.  I can tell you that about $40K was submitted to UHC for the doctor's portion, but I had 3 procedures done...lapband removed, part of my omentum removed and rny.   The hospital portion was about $67K.  Dr. F is GREAT, so I hope everything works out for you!

While I did not have the same surgeon, I do have Tricare Prime and did not require prior payment.  I would call Tricare and inform them of what the surgeon is doing, because this might violate some of their rules and he will get in trouble.

I used the Kane Center, they are non network but because there are no in network surgeons in IL for WLS, I fought and won this arguement and it was treated as in network.
