I got approved!
I have Employee Benefits Manager Services as my insurance. The company I work for is self insured and (apparently) very supportive of weight loss surgery. The biggest issues I had were 1) my insurance changed the day after I had my first appointment with Dr. Rantis, so it has just been a confusing process. I had to get a letter of support from my previous PCP because she was there for my 5 year medical history and she did not want to write anything without seeing me again but her office did not accept my new insurance. I also had to get letters from my new PCP but my old PCP's office took almost 3 months to send medical records..that was crazy enough. The second issue was the amount of tests that my insurance required before my paperwork could be submitted. It took 4 months for me to complete all of the tests, but the nice part of it is that I only have 2 things left to do before my surgery.

Success begins with wise choices I make,I will not let noncompliance be my mistake, The surgery I had was to assist me in changes. My compliance is for a success long range, So I will think before I decide to cheat. Remember the goals I want to meet. For this surgery is not something I was force to do It was what I wanted for a healthy life!
