Can anyone help me with a letter to the insurance compay?

I went to the Nut. on Friday. from what i know that was my last appointment.I called the surgeons office to find out if there is anythng else I need to do. The lady didn't call me back . that's fine though I'll call back tomorrow. But anyway I have a quick question. Does anyone have an outline of a letter that u wrote to the insurance company. I don't want to sound pathetic or like i'm begging. please pm me with a letter. I want to have everything done and be able to submit this month.

Are we supposed to send in a letter when they submit everything? or does it depend on your insurance? I am still doing my 6 month diet so I am sorry I am no help.

No you don't have to, but i want to. I've seen ppl say they wrote a "heart warming" leter it it helped.
