not a benefit

has anyone gotten approved after surgery wasnt a covered benefit?

Actually I did. I have medicare as my primary and there was never any problem with them, but BCBS was my secondary and as I didn't do their required 12 month supervised diet they did not approve me prior to surgery. And yet they paid for everything and have been paying for all my post-op follow stuff as well. Go figure
