Hey I was wonjdering to all of Doctor Oakleys patients what medical documentation did he require before consult? I got the form in the mail of a checklist I have to do before I get a consult with him so I am kind of curious as to what medical records I need to provide that way if I need more records then just from my pcp I can get them. All the form says is Medical Records.

Just bring whatever you have from your PCP. You'll be fine. He is awesome!

Hmm, I don't remember having to bring in anything other than the form they sent you. Course, my consult was last summer. What was on the checklist?
