Need Some Help With Paper Work For The Dr

I'm filling out paper work for the Dr & im confused on 2 of the questions, this is how its stated Disabled-reason for disability ________________ Onset of disability ____________________ I 'm just confused on what them mean for both Thanks Janelle

I would imagine that it means, if you are disabled, why and when. If you are not disabled, then the answer would be N/A.
