BCBS Alliant Select

I've read the requirements BCBS Alliant Select has for approval of WLS. Most everything I read from anyone with this insurance is that it was simple and approved within a week, etc. I'm just wondering if anyone was approved WITHOUT the 3 years supervised (but failed) diet history? I haven't even had insurance for most of the past 3 years, so I don't have many medical records that are recent. Can you tell me what your experiences were if you have this insurance as for what kind of paperwork was needed for your approval? Thanks

I have this insurance and this is what I submitted for past diet programs.....I had a letter from my PCP stating that 5 years prior to the date I was applying for this surgery, I consulted him about weight loss. Then, I listed all of the weight loss diets I have tried, and how much I lost. Example: Weigh****chers, Nutrisystem, etc etc. I DID not have 3 years of weight management with my doctor, per se, but I had consulted with him at one point and tried some diet pills. He didn't follow me through that, because I didn't stay on the medication very long. I was approved in one week. Granted, this was in 2003 - I've heard that things have changed when it comes to this surgery and insurance. Good luck! Kim Gastric Bypass - Open RNY - April 7, 2003 85 lbs. lost - now at goal weight

Iowa City will give you a paper to list all your diet attempts on, including pounds lost and gained. You will not need medical records, just your best estimates. I don't think they will approve you without it, as it is a requirement.

I filled out the sheet from Iowa City. I didn't know if that was sufficient, or if they required actual medical records regarding diets.
