Stacy- My insurance company also required proof of past medically supervised dieting attempts. I wrote a list in chart form of all the diets I have ever been on that I can remember, approximate dates, weights, duration, how much lost, etc.. One thing the doctor's office should have a record of throughout all the attempts is your weight. I summarized the chart noting how much weight I had collectively lost over my lifetime(300#) and pointed out that my ending weight was over 150# greater than my weight was at my very first diet. (I went back over 20 years!) I also noted that during this time I had been seen by doctors who "advised" me about losing weight and/or I "consulted" with them about various diet plans. I have only been seen once or twice in 20 years specifically related to obesity, however I have several comorbities directly related to it. This seemed to be enough because I got approved within two days. It seems your doctor needs to be more empathetic about your predicament and provide more support. He can have a big influence on the decision even tho he may say he doesn't. Keep trying- it is not a lost cause; I have a feeling you are SOOOOOO close !!!!!

Stacy, I was so worried about having to prove it, too. When BCBS asked for more info from my PCP, I called her and asked her to reference the fact that every time I set foot in her office, I was weighed. On the first visit to her, she mentioned WLS and told me she would help if I ever wanted to go there. I asked her to reference that. In the last three years, I went to a diabetes educator and nutritionist for 6 months and I asked her to reference that. I also, at her request, tried Xenacal for 2 months and I asked to reference that attempt. I also asked her to mention that every year at my annual physical, she would set a small goal like 10 pounds loss over a year's time. I didn't ever meet those goals, but they were in the records and I asked to reference that. Once BCBS got that report, I was approved immediately. You will get there. I'm am getting there at a snail's pace but I should (X my fingers) have MY DATE next Monday or Tuesday! Have a good weekend. Susan
