some help please

Hello I spoke with Melody at Mercy on Friday. She informed me that I need to document my diet history for the past five years and get all my medical records that have documented my state of being overweight. Simple enough. Here is my problem, as I sat down to write down my Diet History I am stuck. I dont know what form to use, or what details to include. Does anyone have a sample letter that I could see. Honestly I've done so many diets the past six years that I dont remember every date, nor every try. What do I need bare minimum? Do you think its horrible that I dont remember every little date or detail? Thanks in Advance Amanda

Amanda, Just put down anything and everything you have ever tired to do to lose weight, including diet pills, drinks etc. Also just estimate on the dates. They don't have to be down to the very day. Good luck with your journey. Stephanie

Amanda, I think just writing it down, any Weigh****chers, any over the counter pills, any prescription pills, slim fast, low carb, low fat, high protein, literally anything you have done. You don't need to be exact on your dates. Also, put the estimated amount of weight you lost when doing each thing. I had documented back to my senior year in high school when I did Diet Center! So, 20 years of diet's failing, bulimia,etc... I also provided my medical records for the past 9 years and highlighted everywhere it said overweight or obese or the like. Good luck. Jillybean
