Help with an Appeal

I was denied my request for surgery, stating additional couseling is needed, after attending 10 sessions with the Behavioral Health Therapist, I feel that the therapist should have told additional sessions were needed me instead before denying my request for surgey. Can anyone help me?

It happened to me when I was done he wrote insrance company a letter and I was approved stick in there I know how it feels
